Local volunteers on Community Councils use their knowledge and experience to target funds where the needs are greatest. To make the best use of contributor dollars, United Way holds agencies accountable for their programs, Grants are typically awarded for a period of three years, with continued funding dependent on satisfactory outcome reports. Adult Dental - Family Service Center

The adult dental program received funds to provide preventive and restorative dental clinic for low-income, uninsured adults in Richland and Lexington counties.  In addition to filling an identified dental health gap in the community, they will assist patients in improving overall health by integrating dental care with healthcare through coordination with local health service agencies.  This program should increase the awareness about the importance of routine oral health among the medically under-served population. With the number of adults who have both a medical home and dental home increasing, the program expects to deliver improvement of overall health status.

Primary Care & Chronic Disease Management - Free Medical Clinic

This grant will assist The Free Medical Clinic in providing no-cost ongoing primary care, medication, disease management and patient education to residents who cannot pay for these services and who do not have health insurance.  The Access to Care and Disease Management program also provides referrals to specialists for those patients needing additional care.   Anticipated outcomes include sustained or increased numbers of uninsured, low- income adults who receive primary care and referral to specialty care, effective patient management of chronic diseases like diabetes, high blood pressure and others and early identification of patients predisposed to these medical conditions.

Social Rehabilitation - MIRCI

MIRCI's Social Rehabilitation Program provides quality treatment services (medication management, coping skills, peer support groups, symptom management, and individual counseling) to adults with serious and persistent mental illness.

Adult Day Health Care - Newberry County Council on Aging

This program provides adult day care for residents of Newberry County.  The program allows individuals to age in place and avoid costly placement in nursing homes or other institutionalized settings.  Services include health monitoring and administration of medicine, supervision of personal care, a structured daily exercise program, group activities, nursing services, individualized activities and nutritious meals.  Anticipated outcomes include maximized independence of older citizens in the community and decrease isolation and related conditions that affect seniors' optimal health and well-being.  

Wellness Center Program - Senior Resources

This grant will provide activities that allow seniors to remain active and independent.  The program activities include congregate dinning, transportation, health and fitness activities, socialization, recreation and volunteer activities. Anticipated outcomes include maximized independence of older citizens in the community and decreased isolation and related conditions that affect seniors' optimal health and well-being.

Year:
The year refers to the first year in which the agency receives funding for that particular program. Grants are typically awarded for a three-year period. Very successful programs may be extended beyond three years and programs that do not meet expectations can be ended before the three year grant cycle is complete.

Amount: The amount refers to the funding the agency receives per fiscal year for the term of the grant.

Reports: Reports refer to the outcome reports agencies file with United Way.  Trained volunteers review the reports for progress. Continued funding for programs depends on satisfactory outcome reports. EE - Exceeds Expectations, ME - Meets Expectations, NI - Needs Improvement, DN - Does Not Meet Expectations
